Financial Information

We aim to offer payment options that meet the needs of all of our patients. Below you will find information on our financial policies, as well as a list of insurances and payments we accept. If you do not see your preferred method of payment, please call our office to find out what options may be available.

Payment is due at the time of services rendered.
Regardless of insurance coverage, you are responsible for any outstanding charges. If you have any billing or account questions, we encourage you to give us a call.

Payment Options:

  • Cash

  • Check (Note: a $40 NSF fee will be assessed for all returned checks)

  • Credit Card - Visa, Mastercard, American Express

  • Financing available through CareCredit

For Patients with Insurance:

As a courtesy to our patients, Viridian Oral & Maxillofacial Surgery will bill your insurance company. We do our best to estimate in advance what your insurance will contribute toward the cost of your procedure and maximize the benefits available to you. Please note that any estimate is not a guarantee of coverage. Any treatment, deductibles, or copays not covered by your insurance are your responsibility on the day of service. If your insurance company doesn’t pay as estimated, you will be billed for the remaining balance. If they pay more, you will receive a refund.

Dr. Sonstegard is currently an in-network provider for the following carriers. This is not a comprehensive list. If you do not see your insurance carrier listed, please call our office.

For Patients Without Insurance:

If you do not have medical/dental insurance that will cover the procedure, we ask for payment in full at the time of service.

If alternative financial arrangements are necessary, please discuss this with the front office staff before treatment is started.

No-Show & Cancellation Policy

Your appointment has been reserved for you. If you are unable to keep your appointment, we require 48 hours notice for cancellation and/or rescheduling. If 48 hours notice is not provided, a cancellation fee will be applied to your account.

Late Payment & Finance Charge

All balances over 60 days past due are subject to monthly finance charges. Balances over 90 days past due will be reviewed for collection proceedings.
